Cartagena vs Medellín is the first choice most people planning Colombia face, and for our readers it carries a second question: where would I rather spend the week or two after a procedure? The cities could hardly be more different — one a walled Caribbean port, the other a mountain metropolis — and the differences that matter to a patient are mostly about heat, humidity and getting home.

Medellín’s mean temperature is 23.2 °C, with average lows of 18.2 °C and highs of 28.1 °C, and about 1,685 mm of rain a year.¹ That spring-like steadiness comes from its altitude — about 1,495 m.³

Cartagena, at sea level on the Caribbean, averages about 27 °C, with daytime temperatures of 31–33 °C and nights that stay at 24–25 °C.² It is humid, and there is little relief after dark.

Will I feel the altitude in Medellín?

Most visitors don’t. The city where altitude more often comes up is Bogotá, at about 2,640 m.⁴ The US State Department notes that many Colombian cities, such as Bogotá, are at high altitude, and points travelers to CDC guidance.⁶ If you have heart or lung conditions, ask your doctor about altitude before booking either Medellín or Bogotá.

03Airports and US nonstop flights

Medellín (MDE) — José María Córdova airport is in Rionegro, about 21–22 km by road from El Poblado, 25–40 minutes via the Túnel de Oriente. As of September 2026, nonstops run from Miami (American, Avianca), Fort Lauderdale (Avianca, JetBlue), New York JFK (Avianca), Orlando (Avianca, Frontier) and Houston (United).⁵

Routes change often. Confirm the schedule for your dates, and for a surgical trip prefer a nonstop home — a missed connection is a real problem when you’re recovering.

05Recovery considerations: heat, humidity and rest

For patients, climate is the deciding difference. After body contouring, tummy tucks and liposuction, surgeons usually ask patients to wear compression garments for a period after surgery. In Cartagena’s 31–33 °C days and warm, humid nights, that is uncomfortable, and heat makes swelling and fatigue harder to manage. You will likely spend the recovery indoors with air-conditioning.

Medellín’s mild climate makes short walks — which surgeons often encourage — easier, and air-conditioning less essential. Against that, Cartagena’s old city is flat, while parts of Medellín, including much of El Poblado, are hilly.

Should I avoid the beach after surgery?

Usually yes, at first: sun on fresh scars and swimming before incisions have healed are commonly restricted. Ask yours for specific dates. If a beach week is the point of the trip, schedule it before the procedure or on a later visit.

06Dental and plastic surgery options in each city

Medellín has more clinics that publish prices for US patients among those we track, for both dental work and plastic surgery. It also has several JCI-accredited organizations — Hospital Pablo Tobón Uribe, IQ InterQuirófanos, InSer and CLOFÁN in Medellín, and Centro Oncológico de Antioquia in neighboring Envigado.⁷

Cartagena also has clinics that publish all-inclusive packages for foreign patients, some with recovery suites. The JCI lists no accredited organization in Cartagena.⁷ JCI accreditation is voluntary and not the only mark of quality, but it is one independent check.
