BBL in Colombia: the published prices, and the safety questions that matter more

The Brazilian butt lift — liposuction, then the harvested fat grafted into the buttocks — is one of the procedures most closely associated with Colombia. ISAPS counted 27,360 buttock augmentations there in 2024⁸. It is also the aesthetic procedure with the most carefully studied mortality risk, and that risk depends heavily on one technical choice the surgeon makes.

This guide covers what a BBL in Colombia costs according to the figures that are actually published, what those figures include, what the safety research says, and the specific questions to put to any surgeon before you book. The price section is short because the published data is thin. The safety section is long because it matters more.

01What does a BBL cost in Colombia?

We could not find a single clinic in Colombia that publishes a standalone BBL price on its own site. The Medellín surgeon whose 2026 guide prices most procedures does not list a BBL at all¹¹. What is published:

• A lipo 360 + BBL package at $7,780 ($8,730 for higher BMI) from Premium Care Plastic Surgery in Cartagena, as listed on Medical Tourism Co. It includes a private recovery suite, transfers, garments, medications and follow-ups².
• A $4,500–8,500 range stated by a Medellín directory site — not a clinic — for surgeon, anesthesia, facility and standard post-op care¹.

The US comparison needs its caveat: the ASPS average of $7,264 is the surgeon’s fee only, and excludes anesthesia, the operating room and other costs³. The Cartagena package includes those and a week or more of logistics. On paper the numbers look close; in total cost, they are not.

Because the BBL is priced as a combination, ask for an itemized quote that separates the pieces:

03The fat-grafting safety record

The specific danger in a BBL is pulmonary fat embolism: fat injected into or beneath the gluteal muscle can enter the large veins there and travel to the lungs. The research record has three landmarks:

1. 2017ASERF survey. Surgeons reported 32 fatal pulmonary fat emboli over their careers, a career mortality of 1 in 6,214, plus 103 nonfatal cases⁵.
2. 2018Multi-society task force. Five plastic surgery societies, including the ASPS and ISAPS, put the death rate at approximately 1 in 3,000 — “the highest for any aesthetic procedure” — and told surgeons to graft only into the subcutaneous space⁴.
3. 2024After the change. Among US cosmetic surgeons, 22,800 cases in 2016–18 had 2 deaths (1 in 11,400). In 2019–21, 12,172 cases had zero pulmonary fat emboli, as the share injecting only above the muscle rose from 39% to 82%⁶.

The lesson from the data is clear and it applies in any country: the placement of the fat is the variable that moved the numbers. A BBL in Colombia done with subcutaneous-only grafting by a trained plastic surgeon sits in a very different risk category from one done by a non-specialist in an unregistered facility. In the US, where the procedure is also popular, the ASPS counted 25,662 BBLs in 2025, down 5% on the year before⁷.

04What to ask the surgeon

These questions come straight from the task-force recommendations⁴. A qualified surgeon will answer them without hesitation; an evasive answer is informative.

1. Where exactly does the fat go? The answer you want is “only in the subcutaneous layer, above the muscle”.
2. How do you track the cannula tip? The task force says the surgeon should watch the tip position on every stroke and avoid downward or deep angles.
3. What cannula do you use? The task force advises against bendable cannulas.
4. Do you inject only while the cannula is moving? That is the recommendation.
5. Would you stage a large graft? The task force prefers staging over deeper injection when more volume is wanted.
6. Do you use ultrasound guidance? Some surgeons use ultrasound to see where the cannula sits relative to the muscle. It is a reasonable question to ask; practice varies.

Ask too who administers anesthesia, where you will be operated on, and whether that facility can handle an emergency or has a transfer arrangement with a hospital.

06Recovery and the flight home

The ASPS gives no BBL-specific day count. Its general guidance is to wait five to seven days after body procedures such as liposuction before flying¹⁰. The Medellín directory site says most BBL patients fly home at about two weeks, with their surgeon’s clearance¹. The CDC adds that both surgery and air travel raise the risk of blood clots⁹, which argues for the longer end.

07How to verify the surgeon

Confirm that the surgeon is registered as a plastic surgeon in ReTHUS — you will need their cédula number — and look for them in the SCCP member directory, filtered by city. 08Risks, and what recourse you have

Beyond fat embolism, the risks include infection, seroma, blood clots, contour irregularities and loss of grafted volume. The US State Department’s Colombia advisory (Level 3 — Reconsider travel) notes that US citizens have suffered serious complications and died from cosmetic surgery, and advises researching the recovery facility as well as the doctor¹². The CDC warns that patients treated abroad might not have the same legal recourse as at home⁹, and the ASPS notes that packages may include limited follow-up care, if any¹⁰.
